Checklist — What You Need to Prepare

For everyone

• Government photo ID
• Proof of address with your name (recent utility bill or statement)
• Clear photos or short videos of the issue
• Dates you reported the problem and to whom
• Best phone, email, and times we can reach you

Evidence guide

Photos and video

• Take one wide shot and one close shot for each issue
• Use natural light when possible and avoid filters
• Include something for scale when helpful, like a coin or ruler
• Capture labels, serial numbers, or equipment panels if relevant
• Add a short clip to show leaks, noises, or flickering power

Dates and notes

• List the first date you reported the issue and every follow up
• Save texts or emails related to the problem (screenshots are fine)

Document and file tips

• Accepted file types: JPG, PNG, PDF, MP4, MOV
• Suggested file size: under 25 MB per file
• Name files clearly: address_issue_date (example: 123Main_mold_2025-01-20)
• Scan with your phone using the built in “Scan” feature or a scanner app
• Do not edit or alter the images beyond basic cropping for clarity
• If you need help uploading, tell us in your intake and we can assist

If you are missing a document

Do not stop your intake. Provide any substitutes you have.

• No lease: upload rent receipts, a letter from the owner, or a utility start order
• No utility bill: upload bank or benefit mail with your name and address
• Unsure of tax status: upload the most recent tax notice or a screenshot of the county portal
• No photo ID: upload another photo document and tell us what is missing
